Centerstone Case Manager
- Nancy Caple, a case manager/service coordinator at Centerstone, will be on-site at Downtown Clinic.
- We can refer patients to her directly for help with case management for children who:
- have chronic or complex medical problem
- getting therapy/services w Centerstone AND
- need help with resources.
- "Chronic/Complex medical problem" includes asthma or obesity. The list of options is long, so feel free to refer and they will check if insurance will qualify patient for services. If they have been hospitalized in past 2 years, then more likely to receive services, but that's not needed for referral.
- To make referral, enter as referral to social work and type "NANCY" in the comment box.
- She does not bill same codes as care coordination, so both services can be in place.
