  • 36 months of age and older
    • 90686 - Influenza Virus Vaccine Quadrivalent Split - Prefilled Syringe
    • 90688 - Influenza Virus Vaccine Quadrivalent Split - Multi-Dose Vial
  • Flumist
    • 90473 - Flumist is the only vaccine given
    • 90474 - Flumist is given with other vaccines
